An employee separation notice should identify the problem, list the previous attempts to resolve the problem (noting dates and warnings), and be signed by a company officer or business owner. After reviewing his workforce file, you're astonished his previous supervisor has rated him "above average" on his performance appraisals over the past 4 years.
